Case Management

Case Management at LifeWise provides coordinated, wraparound support designed to help clients navigate care with clarity, consistency, and confidence. This service focuses on collaboration, communication, and continuity, ensuring all aspects of a client’s support are aligned and effective.

LifeWise case managers collaborate closely with a client’s current care providers, including therapists, psychiatrists, treatment centers, schools, and other professionals, to support shared goals and reduce gaps in care. We help organize services, monitor progress, and advocate for client needs across systems.

For clients who do not yet have an established care team, LifeWise offers trusted referrals to vetted providers and community partners to help build appropriate, ethical support from the start.

Case management is ideal for clients who:

  • Are working with multiple providers or levels of care

  • Are transitioning between services or treatment settings

  • Need additional structure, coordination, and follow-through

This service wraps around existing care rather than replacing it, providing oversight, resource connection, and consistent communication so clients and families can focus on stability, healing, and progress.
